Shoulder to Shoulder’s (S2S) mission is to assist high-need individuals and their families in the communities served by Upward Health®. S2S was formed and seeded by Upward Health® leadership in response to observing the urgent non-healthcare needs of Upward Health’s patients and their extended communities.
100% of donations go directly to the foundation’s mission.
S2S supports communities with meaningful one-time assistance to address urgent needs.
Upward Health® provides far-reaching support for patients’ physical, behavioral, and social needs. However, field staff often encounter distressing situations that normal Upward Health’s interventions are unable to address.
The purpose of S2S is to assist with these types of needs for disadvantaged individuals in the communities we serve.
